Dontage Policies

• We require at least 48 hours’ notice for all appointment cancellations or rescheduling.
• To help you remember your upcoming appointment, our scheduling system will send automated reminders via text beginning one week before your scheduled appointment. Reminders will continue periodically until your appointment has been confirmed.
• Cancellations or rescheduling requests made within 48 hours of your appointment will incur a $100 fee .
• All appointments require a credit card on file at the time of booking.
• If we are unable to process the fee using the card on file, an alternative payment method will be required.
• Outstanding fees must be paid before scheduling future appointments.
• Please contact our office directly by phone or text to cancel or reschedule. Instagram and Facebook messages are not accepted methods of cancellation.

We appreciate your understanding, as last-minute cancellations can be difficult to fill and directly affect our providers’ schedules.

Patients who miss their scheduled appointment without notifying our office will be charged a non-refundable $200 no-show fee .

The fee will be charged to the card on file. If payment cannot be processed, the outstanding balance must be settled before any future appointments can be booked.

We kindly ask that patients arrive 10–15 minutes before their scheduled appointment to allow time for check-in and any necessary paperwork.

• Late arrivals may result in shortened consultation or treatment time.
• Appointments will still end at their originally scheduled time to avoid delaying other patients.
• If we are unable to accommodate a late arrival, the appointment may need to be rescheduled, and the $100 cancellation fee will apply.
• Patients who are 15 minutes or more late without contacting our office will be considered a no-show and charged the $200 no-show fee.

Dontage offers a complimentary initial consultation for all first-time patients.

Any additional consultation that does not include treatment on the same day will incur a $200 consultation fee to account for the provider’s time and expertise.

A valid credit card is required to secure all appointments.

If you have special requests or important information regarding your appointment, please contact our office directly by phone, text, or email.

Notes entered during online booking may not be reviewed until the day of your appointment.

To protect patient privacy and confidentiality, Dontage does not provide medical consultations or discuss personalized treatment plans through Instagram, Facebook Messenger, or other social media platforms.

All consultations must take place in person or through an approved virtual consultation.

• Gift cards are non-refundable and cannot be redeemed for cash.
• Gift cards expire one year from the original purchase date.

Dontage treatment packages are intended for individual patients and are subject to the following conditions:

• Packages cannot be shared or transferred between patients.
• Packages cannot be exchanged for other services.
• Package treatments must be scheduled with the original provider.
• All packages expire one year from the purchase date.

All treatments and services offered at Dontage are elective, and payment is due at the time of service.

Pricing for consultations, medical visits, injectable treatments, prescription medications, and other services is non-negotiable. Our pricing reflects the experience, expertise, and quality of care provided by our team.

Patients seeking adjustments, corrections, or reversal of treatments performed by another provider will be subject to an additional consultation or sitting fee.

We add a 3% surcharge to credit card payments to help cover processing costs. This surcharge does not exceed our cost of accepting your credit card. Debit, prepaid, and EBT cards are not subject to the surcharge, even when processed as credit. You may avoid the surcharge by using another accepted payment method. The surcharge will be listed separately on your receipt.

All follow-up appointments and any additional treatments or enhancements are at the discretion of the treating provider. The provider will determine whether a follow-up is necessary and whether any additional charges will apply during the visit.

All treatments, gift cards, and packages are non-refundable and non-transferable.

Clinical photography is a required part of treatment documentation at Dontage. Photographs allow our providers to monitor progress, evaluate treatment outcomes, and compare results over time.

• Clinical photographs are required for applicable procedures and are part of the treatment consent process.
• Photographs are securely stored in the patient’s medical chart using HIPAA-compliant software.
• Photos will never be shared publicly without the patient’s separate consent.
• Patients who decline required clinical photography may be unable to receive treatment at Dontage.

We love children and pets! However, to maintain a safe, relaxing, and comfortable environment for all patients, we kindly request the following:

• Children under the age of 15 should be accompanied by an adult who is not receiving treatment.
• We ask that pets remain at home during appointments.
• Dontage is not responsible for injuries resulting from failure to follow these safety guidelines, to the extent permitted by law.

With your permission, your provider may use Decoda’s AI-assisted tool to audio-record and transcribe your visit and help prepare your medical note. The Dontage Policies documentation signature will be your consent. Your provider will review the note before the note is finalized. You may decline or ask us to stop at any time without affecting your access to care.
